Exam workflow

When ON, once a paper is approved its contents are hidden from the setter (author) and the approver — they see it in their lists but can no longer open the questions.

A leak-prevention measure: after a paper is locked, fewer eyes can see the actual exam. OFF keeps approved papers viewable for reference.
